Purpose

Beyond Back Pain is committed to protecting the privacy and confidentiality of personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

Information We Collect

We may collect personal information including name, date of birth, contact details, emergency contacts, Medicare details, DVA details, WorkCover information, NDIS information, medical history, medications, assessment findings, exercise records and other information relevant to providing Exercise Physiology services.

How Information is Collected

Information may be collected directly from clients, referring practitioners, healthcare providers, insurers, DVA, WorkCover, NDIS representatives and other parties authorised by the client.

Purpose of Collection

Information is collected to provide Exercise Physiology services, communicate with healthcare providers, prepare reports, manage appointments, process payments, comply with legal obligations and support continuity of care.

Disclosure of Information

Information may be disclosed to treating doctors, specialists, allied health practitioners, insurers, DVA, WorkCover, Medicare, legal representatives, funding bodies or other parties where authorised by the client or required by law.

Storage and Security

Client records are stored using secure electronic systems and/or secure physical storage. Beyond Back Pain takes reasonable steps to protect information from misuse, interference, loss, unauthorised access, modification or disclosure.

Access and Correction

Clients may request access to their personal information and request corrections where information is inaccurate, incomplete or out of date. Requests should be submitted in writing.

Retention of Records

Client records will be retained in accordance with applicable legal, professional and regulatory requirements.

Website, Email and Electronic Communication

Electronic communication may not always be secure. By communicating electronically, clients acknowledge and accept the risks associated with electronic communication.

Complaints

Privacy complaints should be directed to Beyond Back Pain. Complaints will be reviewed and responded to within a reasonable timeframe. If a complaint cannot be resolved, individuals may contact the Office of the Australian Information Commissioner (OAIC).
